Organisation notes:

1. Each student MUST print his/her attendance sheet which will return together with filled evaluation sheet either at the and of the course or before the examination.
Always come to a clinical lesson with white gown, some shoes to change and Medical Faculty badge. One missed signature is accepted only to be credited.

Before enrollment to the course, the students are obliged to print out following forms:

Terms for the exam will be available on the SIS until the end of the course.
 

2. Before going either to Stomatosurgery, Cardiovascular Surgery and ICU 1 or 2 one student will take a locker room key and a chip card from The Department of Anesthesiology and Intensive Care secretary building A6, ground floor in advance.
After changing and before going to the lessons the key and the card will be returned to the secretary!!! Students are kindly asked to come and change themselves in advance enough to be ad the departments on time!

Stomsurg
Come to The Department of Stomatosurgery at 8:30 and ask for Dr. Darázs at the OR in the ground floor. Contact phone numbers: 224962149 or 736 255 211.
Urol - ICU
Come to The Department of Urology (Ke Karlovu 6), ICU, 1st floor at 8:30. First, take a locker room key at the ground floor (Medical Card Index Office) and change yourself in the 5th floor. Contact phone numbers: 22496 7884 or 7887.
OG - OR
Starts at 8:00 a.m. Come to the Department of Obstetrics and Gynaecology (Apolinářská 18). Take the left stairs to the second floor (A "Tower") and knock the door signed KARIM. No gowns are needed! Take shoes to change with you, pls. A contact phone number: 22496 7241.
ICU 1+2
Come to The Departement of Cardiovascular Surgery, building A2, 2nd floor at 9:30. No gowns are needed! Contact phone numbers: ICU1 - 22496 2733, ICU2 – 22496 3481 or 82.
CS OR
Come to The Departement of Cardiovascular Surgery, building A2, 3rd floor at 8:00. No gowns are needed! Contact phone numbers: 22496 3473 or 2733.
ICU3
Come to The Department of Anesthesiology and Intensive Care, building A6, ground floor at 9:30, ER "back" entrance (the same floor as the lecture hall, other side of the corridor). No gowns are needed! A contact phone number: 724 918 034.
